Advances in diagnostic techniques, such as neuroimaging and biomarker analysis, enhance the ability to identify Alzheimer's at its earliest stages, often before significant symptoms appear. In parallel, treatment options are evolving, focusing on medications that target the disease's underlying pathology, as well as non-pharmacological approaches that support cognitive function and emotional well-being.

Alzheimer's diagnosis involves multiple approaches, including evaluating symptoms, conducting various tests, and consulting with family members or friends to gather insights about the patient's behaviour. Healthcare professionals assess cognitive skills, personality or behavioural changes, and the overall impact of symptoms on daily functioning. Common diagnostic tools include mental exams, neuropsychological assessments, laboratory tests, and brain imaging techniques.
In June 2024, revised diagnostic criteria for Alzheimer’s disease were released, incorporating recent advancements in research and diagnostics. These new guidelines emphasise using blood-based biomarkers identified through laboratory tests and brain scans, moving away from traditional written memory tests. This shift aims to enable early disease detection, potentially before symptoms manifest, as this stage offers the most promising opportunity for effective treatment.
The updated criteria suggest that patients could be diagnosed with Alzheimer’s based solely on blood tests, even in the absence of memory difficulties. However, some experts caution that the presence of amyloid-beta proteins in the brain and blood does not necessarily predict future dementia development. While medications for Alzheimer’s can improve outcomes for individuals experiencing early memory loss, there are potential risks, including fluid accumulation, swelling, and microbleeds in the brain.
Alzheimer's Disease Treatment Methods
Currently, there is no cure for Alzheimer’s disease, but personalised treatments can offer temporary relief from symptoms. The most commonly prescribed medications are acetylcholinesterase (AChE) inhibitors, which enhance communication between nerve cells in the brain. Three AChE inhibitors—donepezil, rivastigmine, and galantamine—have received approval for Alzheimer’s treatment. While they function similarly, the choice of medication may vary based on the associated adverse effects.
As Alzheimer’s disease advances, nerve cells become increasingly damaged and may produce excessive amounts of glutamate. Memantine is recommended for patients with severe, late-stage Alzheimer’s to mitigate the effects of excess glutamate. Common adverse effects of AChE inhibitors include loss of appetite, nausea, vomiting, and diarrhoea. In contrast, memantine's side effects tend to be less severe but can include dizziness, headaches, and raised blood pressure.
Alzheimer Disease Treatment Updates
Significant advancements have been made in Alzheimer’s treatment, including the FDA’s approval of lecanemab (Leqembi). This medication has reduced disease progression by targeting amyloid plaques in the brain. Following successful phase 3 clinical trial results, lecanemab transitioned from accelerated to traditional approval in July 2023.
Common adverse effects of lecanemab include infusion-related reactions, amyloid-related imaging abnormalities (ARIA), and headaches. In July 2023, the FDA also approved donanemab (Kisunla), demonstrating significant efficacy in reducing clinical decline in Alzheimer’s patients compared to a placebo. Donanemab is administered as an intravenous infusion and has common adverse effects similar to those of lecanemab.
The Alzheimer's treatment landscape has shifted recently, particularly with Biogen’s decision to discontinue aducanumab (Aduhelm) effective November 2024. Despite its initial accelerated approval in 2021, aducanumab faced significant resistance from medical institutions and healthcare professionals due to safety and efficacy concerns and its high cost of $56,000 per year. Biogen has since redirected its focus toward other Alzheimer's treatment options, particularly lecanemab.
As research advances, the development of innovative diagnostic tools and effective treatment strategies continues to evolve, providing hope for better management of symptoms and a deeper understanding of the disease's mechanisms.
